Parents must call 832-6016 to sign up for a time. Limited Space    

Physical Form:  Please bring the correct OHSAA physical form filled out in
advance.  Forms may be downloaded from the Northmont Web Site or picked up
at the High School and Middle School in advance. 

PAY AT THE DOOR AND PARENTS MUST BE WITH THEIR CHILDREN


SAMARITAN NORTH WILL NOT BE DOING ANYMORE PHYSICALS AT THIS PRICE FOR THE
REST OF THE SCHOOL YEAR.
